Purchase Agreement Finalized

On Sept. 7, 2010, select ProHealth Care Medical Associates physicians, clinicians and clinics became part of the Froedtert Health network. Froedtert Health has established this Web site to provide important information to patients whose care will be transitioned from ProHealth Care.

Letter to Patients

Dear Patient:

We are writing today to share a significant change that will affect you as a patient of ProHealth Care Medical Associates. ProHealth Care has entered into an agreement with Froedtert Health to transition ownership of ProHealth Care Medical Associates clinics, physicians and clinicians in the Germantown, Hartford and Menomonee Falls areas to Froedtert Health.

Honoring patient wishes and supporting the local delivery of health care services has always been part of our respective organizationsí mutual missions. We believe this change of ownership supports those missions and ñ most importantly ñ the best interests of our communities and the patients we serve, many of whom wish to receive health care services close to home.

What does this mean for me?

Beginning Sept. 7, your ProHealth Care Medical Associates clinic became a Froedtert Health clinic. Specifically, the following clinics are now part of Froedtert Health:

• Germantown
• Hartford
• Town Hall Road, Menomonee Falls
• Rehabilitation Services, Menomonee Falls
• Behavioral Health Services, Menomonee Falls

Behavioral Health Services, Wauwatosa, closed permanently on Sept. 7 but these services will continue to be provided at Germantown, Hartford and Menomonee Falls locations.

All other existing ProHealth Care Medical Associates locations and providers will remain part of the ProHealth Care system.

What do I need to do?

Be assured that ProHealth Care and Froedtert Health are working together to plan the seamless transition of your care.

• You will continue to receive care from your current physician.
• Any appointments you have scheduled on or after Sept. 7 at any of the above locations remain unchanged unless you are personally notified by phone of a change.
• Your medical records will automatically transfer with your physician to Froedtert Health on Sept. 7.
