How to Enroll

You can enroll in MyChart® at your clinic. Simply ask for an enrollment form at the clinic reception desk or ask your provider. For patients in the hospital, ask for an enrollment form at the nurse station. After signing the enrollment form, you will be given a code to activate your MyChart® account. When you get home, you log into your computer and create your own MyChart® ID and password. Additional paperwork is required for access to family member health records and appointments.

MyChart® is a free, easy and secure way to view your health information and communicate with your health care team. All you need is Internet access and e-mail. 

You can also take MyChart® with you wherever you go with the MyChart® app*. Visit the Android Market or the App Store on your mobile device or tablet to download the app.
