Advantages of a Medical Home

The Patient Experience

The Medical Home is proven to strengthen primary care because of its focus on patients, managing the health of a larger population, controlling costs and improving outcomes.

The Medical Home enhances and builds upon many of the processes currently in place throughout Froedtert Health Medical Group locations.

Whether you come in for an urgent/same day illness, a check up or ongoing care, your team will proactively address all of your health and wellness concerns at that appointment. That may include scheduling any needed testing a few days prior to your appointment so we can discuss the results and treatment options together. If you are overdue for any testing, screenings or preventive care, you may also receive a letter or phone call that will remind you to schedule those services.

A more robust follow up where your care team proactively reaches out to you to assure your treatment plan is understood, followed and future appointments are made and kept.

A Care Coordinator may work with you after the visit or between visits. They will provide additional patient education, review your plan of care, set healthcare goals, answer your questions and assure that the necessary follow up is in place.

Population Management

Our use of technology, registries, and electronic records allows us to coordinate care and manage large populations or groups of patients, ultimately to improve the overall health status of our patients and community.

By managing the patient registries the care team can focus on a larger pool of patients, that includes healthy patients and those with chronic conditions. This tool helps identify patients that may have fallen through the cracks because they are overdue for care, missed a screening, have a chronic condition, have not followed their treatment plan or did not make follow up appointments.

Controlling costs by improving outcomes and increasing efficiencies

Increased health education improves your understanding of any chronic disease and reasons for health screenings.

Standardized workflows and after and between visit contacts allow for more opportunities for patient education. This results in increased compliance, and therefore a greater likelihood for a better outcome from your treatment plan.
