A new delivery of care

The Medical Home is a model for delivering health care that addresses each patient's full range of health and wellness during all stages of their life. We believe this is an exciting opportunity to transform the health of our community with a more patient-centered, proactive and evidence-based approach.

The purpose of the Medical Home is to provide you with increased health education, resources and follow-up. These evidence-based approaches have been proven to help people experience healthier outcomes. The Medical Home uses a team approach that will help you better understand and manage your treatment plan, medications, chronic conditions, necessary screenings, or any special concerns.

The Froedtert Health Medical Home treats you through all stages of life - both acute and chronic. With the new Medical Home there is an increased emphasis on you and your needs by focusing on:

  • Greater access
  • Patient education
  • Self-management of chronic health conditions
  • Goal setting
  • Prevention
  • Use of evidence-based guidelines for care

Whether you come in for an urgent/same day illness, a check up or ongoing care, your team will proactively address all of your health and wellness concerns at that appointment. That may include scheduling any needed testing a few days prior to your appointment so we can discuss the results and treatment options together. If you are overdue for any testing, screenings or preventive care, you may also receive a letter or phone call that will remind you to schedule those services.

Your Medical Home Journey

The expansion of Froedtert Health Jackson Clinic (July 2010), allowed the perfect opportunity to introduce this exciting new method of care, which has received national recognition for its successes in improving the quality of care patients receive.

Recently, Froedtert Health Jackson Clinic was awarded recognition for their medical home model by the National Committee for Quality Assurance (NCQA). They’ve received Level 3 Physician Practice Connections ® - Patient-Centered Medical Home Recognition (PPC-PCMH™). In addition all of the family medicine providers were also recognized by the Diabetes Recognition Program (DRP).

Froedtert Health Jackson Clinic has served as the medical home pilot site for Froedtert Health Medical Group. Plans are now in place to get all remaining primary clinic sites throughout Washington and Waukesha counties transitioned to the medical home model of care. 

In November 2011, physicians and services from the Froedtert Health Hartford Clinic – West location, 402 W. Sumner St., moved into the Froedtert Health Hartford Clinic – East location, at 110 Lone Oak Lane, forming the current Hartford Clinic. This combination of services presented a great opportunity to develop our medical home care model – which has shown great success in Jackson – into our primary care physician practices at this location.
