Froedtert Health Jackson Clinic was the first clinic in our Medical Group to pilot a new model of care, the medical home. The Medical Home is a model for delivering health care that addresses each patient's full range of health and wellness during all stages of their life - both acute and chronic. Recently, Froedtert Health Jackson Clinic was awarded recognition for their medical home model by the National Committee for Quality Assurance (NCQA). They’ve received Level 3 Physician Practice Connections ® - Patient-Centered Medical Home Recognition (PPC-PCMH™). In addition all of the family medicine providers were also recognized by the Diabetes Recognition Program (DRP).
Froedtert Health Jackson Clinic Expansion
The expansion of the Jackson Clinic has been completed as of July 2010. The expansion allowed for additional family practice services, preventive medicine, women's health, on-site lab, radiology, and EKG services. The expansion added 7,200 sq ft. bringing the total to 17,600 square feet and features 24 exam rooms, 8 physician offices, and procedure rooms.
Froedtert Health Jackson Clinic is a satellite of Froedtert Health Medical Group and offers patients access to many specialty services and more advanced care in situations where it is needed.
