In November 2011, physicians and services from the Froedtert Health Hartford Clinic – West location, 402 W. Sumner St., moved into the Froedtert Health Hartford Clinic – East location, at 110 Lone Oak Lane, forming the current Hartford Clinic. The clinic on Lone Oak Lane was remodeled to accommodate the additional physicians, staff, patients and services, including a newly expanded rehabilitation and physical therapy department. Patients of the Hartford Clinic have the option to use the services of Froedtert Health Community Memorial Hospital in Menomonee Falls or Froedtert Health St. Joseph’s Hospital in West Bend.
This combination of services presented a great opportunity to develop our medical home care model – which has shown great success in Jackson – into our primary care physician practices at this location. The Medical Home is a model for delivering health care that addresses each patient's full range of health and wellness during all stages of their life - both acute and chronic. Services
Froedtert Health Hartford Clinic offers patients primary care services in family medicine and OB/GYN, and specialty services in the areas of behavioral health, gastroenterology, general surgery, neurology, optometry, orthopaedics and podiatry. On-site services include laboratory, mammography, general x-ray and physical therapy. We offer the convenience of same day appointments, urgent care and evening hours. New patients are welcome.
