In August 2010, West Bend Clinic was rebranded as Froedtert Health Medical Group in conjunction with the launch of the new Froedtert Health name. One month later, an additional 50 physicians in Germantown, Hartford, Menomonee Falls and Sussex joined the system, also under the name Froedtert Health Medical Group. Froedtert Health Medical Group is staffed by more than 150 providers and specialists across 16 locations in Washington and Waukesha counties. In addition to primary and multi-specialty care, services also include behavioral health, rehabilitation and outpatient surgery.

Froedtert Health Medical Group brings a commitment to quality clinical care and to improving the quality of our patients' experience, while operating efficiently. Froedtert Health Medical Group will continue to develop sophisticated tools and use an electronic medical record system to assist in ongoing efforts to enhance quality, manage chronic disease and increase patient compliance with necessary testing, preventive measures and screenings.

The Jackson Clinic is one of few clinics in southeast Wisconsin providing a team based “Medical Home” for patients. The Medical Home has received national recognition for its success in improving the quality of patient care. The Jackson pilot has demonstrated initial success in improving the health and wellness of our patients as demonstrated by the measurable quality outcomes. The model will be expanded in fiscal year 2012 to more departments and locations within Froedtert Health Medical Group. In this model, the primary care physician works with a team to coordinate care. The team utilizes patient registries, follows standard workflows, and provides education.

Froedtert Health Medical Group continues to collaborate with Workforce Health in implementing a system wide employer “Total Health” strategy for services at the worksite that will help local employers improve the productivity and health status of their employees, while lowering their health care costs.

To demonstrate our connection and commitment to the community, Froedtert Health Medical Group donated to more than 60 local non-profit organizations and schools. Physicians and staff participate in more than 90 community events, programs and outreach. Many free screenings are offered in the community for asthma, skin cancer, prostate cancer, diabetes, osteoporosis, hearing and vision.
